Please note: The forms below must be printed out, filled in, and returned to our office by mail or in person. We must have a signature on the form to verify authenticity.

 

K-Statement forms -- Noteworthy / Plaudits / Photo release form

Student bio form -- used for submitting student information for hometown publication. Complete and sign the form and return it to Media Relations by mail or hand-deliver to the address above. For currently enrolled students only.

Faculty bio form -- faculty biographical information used for background material for news releases on research, awards and promotions; introductions for speaking engagements; etc. Complete and sign the form and return it in campus mail or hand-deliver to Media Relations at the address above.

Student employment job application form -- application for student employment in the K-State Media Relations office. Please check with Media Relations at 785-532-6415 for student job openings. Job descriptions are available in the Media Relations office, 9 Anderson Hall.
